NEW ITEM AUTHORIZATION FORM

Please read the instruction tab found in the Microsoft EXCEL workbook and complete all fields that apply. List all of your individual UPCs on separate lines. Each SKU (item) will be evaluated individually. Rank your items in the order that they sell, with the best seller listed at the top and the lowest seller at the bottom.

HOW TO COMPLETE THE FORM:

  • Enter your name in the box at the top left of the form.

  • If you have a broker, please enter the name of the brokerage firm (not the person).
  • Enter your company’s primary/parent name. (If your brand name is different than your company name, include the company name.)
  • Enter the New Item Authorization Form submission date.
  • Enter the phone number for placing orders with you.
